Health Office

(1) ISSUANCE OF MEDICO-LEGAL CERTIFICATION          

(a) Essence. There are patients that would like to hold papers for their health condition, for accidents met that brought them to the health center and for other circumstances that need medico-legal action. It is therefore necessary that Medico-Legal Certification be processed and issued according to facts and actual health medication and examination.     

  1. Processing Time. If the requirements are complete, this transaction shall be completed within forty five (45) minutes.

 

  1. Clientele. Barangay residents and people seeking health-related services.

 

  1. Requirement: If the requirements are complete, this transaction shall be completed within forty five (45) minutes.

 

(1)  Police request;

(2)  Community Tax Certificate;

(3)  Official Receipt for Medico-Legal fees.

 

  1. Availability of Service. During office days:   Monday to Friday from 8:00 A.M. to  5:00 P.M.  NO NOON BREAK

 

  1. Fees and Charges. Medico-Legal Certification fees and charges shall be based on the rates stipulated in the existing municipal tax ordinances.

(2)  GENERAL CONSULTATION ON PATIENTS WITH COMMUNICABLE AND NON- COMMUNICABLE DISEASES

     

(a) Essence. One of the priority programs of the Municipal Government being carried out by the Municipal Health Center is the consultation on patients with communicable and non-communicable diseases to prevent the spread of the same. It shall therefore be the responsibility of the Municipal Health Officer to cause the          conduct of consultations on patients suffering from different kinds of ailments.     

 

(b) Processing Time. If the requirements are complete, this transaction shall be completed within two (2) hours and twenty nine (29) minutes.

 

  1. Clientele. Barangay residents and people seeking health-related services.

 

  1. Requirements: If the requirements are complete, this transaction shall be completed within one (1) day and thirty five (35) minutes.

 

(1)  Community Tax Certificate;

   (2)  Official Receipts for:

(a)  CBC;

(b)  Platelet;

(c)  Urinalysis (U/A);

(d)  Stool exam (S/E);

(e)  Sputum;

(f)   Blood Typing.

(3) Sample of urine for processing and microscopic examination of urine specimen and recording;

  1. Stool specimen for stool examination.

 

  1. Availability of Service: During work or office days:  Monday to Friday  from 8:00 A.M.  to  5:00 P.M.  NO NOON BREAK.

(3) MEDICAL CONSULTATION AND DIAGNOSIS ON PATIENTS SUFFERING FROM TUBERCULOSIS PURSUANT TO THE NATIONAL TB PROGRAM

 

(a) Essence. The National Government intensifies its medical drive against the spread of tuberculosis. Under the National TB Program, the Municipal Health Center conducts consultation and diagnosis on TB victims. For six (6) to twelve (12) months, the Center conducts daily directly observed treatment of short course chemotherapy if only to deliver the TB patients from their state of suffering.   

     

  1. Processing Time. If the requirements are complete, this transaction shall be completed within three (3) hours and 34 minutes plus 6-12 months for daily Directly Observed Treatment of Short course Chemotherapy.

 

  1. Clientele. All citizens residing in the Municipality of Anilao seeking health-related services.

 

  1. Availability of Service: During office days:  Monday to Friday  from 8:00 A.M.  to  5:00 P.M.  NO NOON BREAK.
